You do great portraits, Mal; this guy looks very assertive, and so I can understand "noisy". I like the big fist in the foreground, and love the light caught in the rather sparse hair on his left scalp. Actually....if this guy is old then I must be ancient....ha ha!
The way you managed to capture the look in his eyes does this portrait...

Joking aside, Mal, it's a cracking portrait. The digital noise complements the smoky atmosphere for a gritty portrait with boatloads of character. Nice one.

This is rather special, Mal. Everybody else has already mentioned what makes it so special, so I won't repeat. I'll just say that I've sat here for an inordinately long time soaking up the atmosphere. Really something special.

As always your posted portraits tell a story or two. I agree with pixelpixie about the background. At least your subjects are consistent and they all are trying to cut back by smoking small cigarettes.
